Hospitals, nursing homes, and health care agencies have their own team that meets on a regular basis to follow up on your conditions medically, mentally, and physically and provide medications and rehabilitation. We provide you the choices of physicians, nursing homes, assisted living, board & care, hospice care agencies, home health agencies, and caregivers.
Upon admission or acceptance to the place and the type of services you want, the facility care team and the home care representatives will discuss the plan of care for you. The medical director, nursing director, social worker, dietary director, registered nurse, and rehabilitation director will discuss the best care planning for short-term or long-term care for you. The business office manager will discuss your Medicare or Medical insurance benefits. Your attending physician can still follow up with you anywhere you go as long you want your attending physician to do so. The facility or servicing agency can provide you with another doctor of your choice.
Your care team will respect your privacy, dignity, and your rights. We encourage family members and friends to get involved with care for emotional, spiritual, and mental support to maximize your quality of life fully.

PRIMARY SENIORS CARE TEAM
Skilled Nursing Facility (SNF)
Attending Physicians, Rehabilitation Team -(RPT) Registered Nurse (RN) LVN, Dietary Director, Social Workers, Case Managers ,Admissions and Activity director with the supervision of the Hospital Administrator. Accepting Medicare, Medicaid, HMO, PPO, ACO ,Private health insurance and LongTerm Care Insurance.
Assisted Living Facility (ALF)
Patient/family pays for the Room & Board Medicare -Part A & B Medicare Advantage pays for Hospitalization, Doctors, Medicine and other services, some with co-pay. Medicaid pays the Room & Board, Medicines, Rehabilitations and Meals (You may ask the Assisted Living what are included) Care Team are Administrator, Director of Nursing, Social Services, Rehabilitation team, Dieatry , Nursing Assistant and caregivers and Attending physician.
Board & Care Facility
All patients are Private pay ( Room & Board different Rates) Meals, Nursing Assistant & Caregiver, Acitivities of daily living (ADL) are included. Care Team are Residential Care Facility (RCFE) administrator and Caregivers. Attending physicians are on call at any time.
Hospice Care Services
To qualify for Hospice care, a hospice doctor and your doctor must certify that you are Terminally ill, meaning you have a life expectancy of 6 months or less, When you agree to Hopsice care, you’re agreeing to Comfort care (Pallative care) instead of care to cure your illness. Hospice care team are – Registered Nurse (RN) LVN, Nursing Assistant (CNA) Certified Home Health Aide (CHHA) Social Services, Chaplain and Volunteers.
Home Health Care Providers
Agency services providing Continuoeus & Routine Care visitation to Patients Home, Assisted Living and Board & Care facility Medicare covers home services under Medicare Part A&B. Medicare covers Medically necessary part-time or Intermittent Skilled Nursing care, Physical Therapy, Speech therapy or continued Occupational therapy services. Home health may also include medical equipments and Medical supplies. Care Team are -Registered Nurse, LVN, Cerified Home Health Aide (CHHA) or Caregivers (Private Pay).
Caregivers Providers
Caregivers are all Private pay basis some patient with Long Term Health Insurance for the services for Mobility /Transfering, Dressing, eating, Grooming, Toileting, Continence care, Meal preparation, Medical reminders. Companionship, Monitor Vital signs, Light Housekeeping, Medical Appointments, Bed Making and some provide Transportatiom. Caregivers must be Certified Nursing assistant, Experience Caregivers, some are Multi-lingual, Good moral character and criminal background are check before hired. Working hours can be arrange to family and agency services.
